Support Coordination

We can help you get the best out of your NDIS plan with our Support Coordination service connecting NDIS participants with informal, mainstream and specialist services. Your support coordinator will seek to really understand your goals and will explore with you what supports you need to achieve them. These may include paid services, activities available to anyone in the community, and identifying friends and family who can be a part of your support system. 
 

We work with you to build independence and improve quality of life and support you overcome any barriers that you may encounter. A trained Support Coordinator will work with you to ensure you have access to the required supports to achieve the goals outlined in your NDIS plan.

Complete care coordination will help you navigate the service system and can assist you to develop service agreements with providers. Complete care believe in empowering people to take control of their lives, and this means we will help you build the skills and confidence to manage your own plan in the future.

We offer three variations of support coordination to people with NDIS funding, including:

  • Support connection - where we assist you to connect to NDIS-funded and mainstream services

  • Coordination of supports - where we also coordinate your scheduled support services

  • Specialist support coordination - for those who have multiple and complex support needs and want assistance to negotiate specialist support.

Complete care coordination offer NDIS Support Coordination support across Shepparton region and the Goulburn Valley areas as well as providing remote assistance if you require our services and aren't within the region. 

Specialist Support Coordination - Mental Health / Psychosocial
Specialist Support Coordination is high-level support coordination designed for those experiencing complex support needs for their mental health conditions being the primary focus. Our support coordination staff have a background of a Diploma in Mental Health as well as life experience to help understand the ways your mental health can affect all other areas of your life and help you get back on track. Specialist Support Coordination will assist you with the following:

  • Connection with supports to meet your needs;

  • Ensure your NDIS services are delivered in a consistent way;

  • Address any barriers in your current situation and environment;

  • Assist you to build greater capacity and resilience.
